Liz Dunleavy, MSW, LSW, CMC
Liz Dunleavy, of Kith Elder Care, serves as a Geriatric Care Manager (GCM) in the greater Philadelphia area. Liz is a licensed social worker and certified care manager. With her experience, she provides expert guidance, caring advocacy and timely connections with cost-effective services for older adults and their families, planning for current and future needs.
Liz received her Master's degree in social work from Fordham University with an emphasis in gerontology and her Bachelor’s degree in psychology from Simmons College. She is a member of the Aging Life Care Association (ALCA) and serves on the board of its Mid-Atlantic Chapter. In addition, she is co-chair of the ALCA Philadelphia Unit.
Bonney Dahlgren DosSantos, BSW, CMC
For over 20 years, Bonney has worked as an advocate for older adults and those with disabilities. Much of her career was spent as a geriatric care manager for Catholic Housing and Community Services of the Archdiocese of Philadelphia. In her time there, she founded and developed the Saint Monica Eldercare Program, which became a model for care management and service provision throughout the Archdiocese. Bonney also has a broad knowledge of Medicare and enjoys advising individuals in choosing insurance that best meets their needs. Since retiring from full time employment with CHCS, Bonney continues her work as an advocate with Kith Elder Care, where she continues to follow her motto - "Supporting Choice, Providing guidance".
What is a Geriatric Care Manager (GCM)?
A professional Geriatric Care Manager (GCM) is a health and human services specialist who helps individuals as they care for older friends or relatives. The GCM is trained, experienced and certified in any of several fields related to care management, including social work, nursing, and gerontology. The GCM uses a holistic approach to create a plan of care that encompasses multiple facets of the client's life. Typically the assessment is followed by ongoing care management that provides oversight of the client and any services they may be receiving. Through ongoing care management, changes in status are more readily identified and addressed.
